For the least three decades, nurse practitioners (NPs), certified nurse-midwives (CNMs) and others nurses with advanced education and skill have provided primary health care to many persons, including those who otherwise would have been undeserved or without health care. Considerable research has been generated   on the acceptability, cost effectiveness, and substitutability of these providers for physicians in the provision of primary care.
